Services provide personalized and confidential choices for meeting the needs and lifestyle interests for you or a family member.

Medicare/Medicaid Assistance

Available when older adults want objective counseling, education and enrollment comparisons when selecting Medicare Part D prescription or Advantage programs. This past year, over 350 individuals were personally assisted with benefit choices, filling out forms and enrollment questions resulting in thousands of dollars saved in out-of-pocket costs. The open enrollment for this programs runs from mid-October through early-December. Certified Medicare/Medicaid staff and trained volunteers provided 1,500 hours of service resulting in 1,185 contacts with over 240 older adults this past year.

Prescription Assistance 

Helps low and moderate income older adults gain access to no-cost or reduced-cost medications. Over $100,000 worth of prescriptions are obtained through this program each year. Staff dedicated over 700 hours to obtain over $83,700 in medication value for those experiencing financial challenges.

Tax Assistance

Provides free tax preparation service from February through April. Over 400 individuals receive more than $200,000 in refunds or previously unclaimed tax credits annually through this program. Dedicated volunteers and staff served over 400 individuals and obtained over $210,000 in refunds or previously unclaimed tax credits.

Emergency Unmet Needs

Program that assists older adults who have limited financial means with medication expenses and other basic needs so they may continue to live safely and independently. This year, over $10,000 was directed to provide solutions to emergency situations and restore clients’ independence.

Today is National Walking Day!!!

While the snowy view out our windows may not be particularly motivating, there are many ways you can still step out and get moving. Did you know Four Pointes has an indoor walking route? 15 laps inside our building is equivalent to 1 mile! Whether you walk inside or out, long or far, stay close to home or set out on an adventure.....all steps count!!!

We are looking forward to kicking off 8 weeks of walking with the Step It Up Fitness Challenge with OC Department of Public Health and Ottawa County Parks and Recreation next week!!! ... See MoreSee Less
