Services provide personalized and confidential choices for meeting the needs and lifestyle interests for you or a family member.

Medicare/Medicaid Assistance (MMAP)

Available when older adults want objective counseling, education and enrollment comparisons when selecting Medicare Part D prescription or Advantage programs. This past year, over 350 individuals were personally assisted with benefit choices, filling out forms and enrollment questions resulting in thousands of dollars saved in out-of-pocket costs. The open enrollment for this programs runs from mid-October through early-December. Certified Medicare/Medicaid staff and trained volunteers provided 1,500 hours of service resulting in 1,185 contacts with over 240 older adults this past year.

Prescription Assistance

Helps low and moderate income older adults gain access to no-cost or reduced-cost medications. Over $100,000 worth of prescriptions are obtained through this program each year. Staff dedicated over 700 hours to obtain over $83,700 in medication value for those experiencing financial challenges.

Emergency Unmet Needs

Program that assists older adults who have limited financial means with medication expenses and other basic needs so they may continue to live safely and independently. This year, over $10,000 was directed to provide solutions to emergency situations and restore clients’ independence.

AgeWell will continue to distribute meals TO GO from Four Pointes kitchen for the month of July on Mondays, Wednesdays, and Fridays-11am-1pm.

In addition to the hot meals, we are excited to be adding salads and cold sandwiches to the menu!

**PLEASE NOTE:**You must call ahead to RESERVE a meal by 12-noon of the prior AgeWell service day. For example, if you’re interested in the meal on Wednesday you need to call and reserve the meal on Monday by 12-noon. Reservations can be made FAR in advance and set for the whole month of July. You cannot pick up meals for someone else. Meals limited to two per person.
