Eligibility:

  • 60 years of age or older.
  • Resides in North Ottawa community served by Harbor Transit.
  • At or below 200% of the annual poverty level.
  • Does not have reliable transportation options, including a vehicle or support system.
  • Live in independent housing (not in assisted living or nursing home.)

We strongly encourage participants to use the vouchers to attend Wellness Center activities to address social isolation, but also recognize that they are for personal use to help an individual remain independent.

Procedure:

  • Each participant will need to fill out an agreement every fiscal year. Once the agreement has been approved by staff, the participant will receive a bus voucher with a code. The voucher is good for 12 one-way rides.
  • The rider initiates the monthly request for one voucher providing 12 one-way rides by contacting via telephone or coming into Four Pointes. A survey must be filled out before distributing the next voucher. 
  • One voucher per month per user. Extenuating circumstances can be discussed with the HR/Volunteer Coordinator.  In his absence, the Director may be conferred with.
