How our senior assistance program works

Our approach is person-centered and practical:

  • A trained team member discusses your goals, budget, and timing.
  • We review available community resources and Four Pointes programs.
  • We connect you with the options that fit you the best, including Four Pointes services, local partners and statewide resources.
  • When available, we help with applications or referrals to speed things up.

We check in, refine the plan if needed and stay available for future questions.

Eligibility and availability

Adults 60+ and family caregivers in North Ottawa County, including Grand Haven, Ferrysburg, Spring Lake, Polkton Township, Grand Haven Township and Robinson Township.

Some supports are referral-based, and some are limited by funding or partner capacity. We will always explore available options with you.
