must ASSESSMENT SERVICES

Programs provided in the home of the older adult who may be experiencing difficulties in living independently. Trained staff meet with the older adult and family members to coordinate services that may be brought into the home for a safer, more comfortable independent living environment.

HOMEMAKING AND COMMUNITY LIVING SUPPORT PROGRAM

Provides support in the home for older adults in northern Ottawa county. Support may include but not limited to housekeeping, errand running, meal preparation, personal care, bathing and companionship from trained staff.

MEDICAL RIDE PROGRAM

Provides safe transportation to and from medical and other necessary appointments at no charge to older adults in north Ottawa county. Rides provided by dedicated Four Pointes volunteers. 

TELEPHONE REASSURANCE

Daily calls to check on individuals who may be homebound.

HOME DELIVERED MEALS

Meals can be arranged to provide daily hot or cold nutritious meals in the comfort of one’s home.

MEDICAL EQUIPMENT

Gently used medical equipment is available on loan for a small fee. Choose from a variety of items including benches, crutches, cane, and wheelchairs.

To see if you qualify please contact us. 

If you would like to make a donation to our loan closet, we ask that you call beforehand. For health and safety reasons, all equipment must be cleaned, sanitized, and in good working condition. Four Pointes reserves the right to refuse items that do not meet these standards.
