Available when older adults want objective counseling, education and enrollment comparisons when selecting Medicare Part D prescription or Advantage programs. Over the years, many individuals were personally assisted with benefit choices , filing out forms and enrollment questions resulting in thousands of dollars saved in out-of-pocket costs. The open enrollment for this program runs mid-October through early-December.
Prescription Assistance
Helps low and moderate income older adults gain access to no-cost or reduced-cost medications. Each year over 100,000 prescriptions are obtained through this program.
Unmet Needs
Assists older adults who have limited financial means with medication expenses and other basic needs so they may continue to live safely and independently.
Resource & Referral
Assists older adults and their families who may be experiencing difficulties living independently by connecting them with services provided through Four Pointes and/or other resources in the community.
Volunteer Transportation
provides safe, secure transportation to medical and legal appointments. Harbor Transit passes are provided to those with limited financial means to enable them to meet appointments and/or run errands.
Telephone Reassurance
Volunteers provide daily calls to check on individuals who may be homebound.
Medical Equipment
Available on loan for a small fee. With a wide variety of items including benches, crutches, canes, walkers, wheelchairs and more.
